8. Application Form

This application is for Southside Church in Peachtree City, GA.

Each person applying for membership needs to complete all parts of the application. (i.e. Four family members desiring membership need to complete four separate applications.) Parents can assist or complete this portion for children under the age of twelve.

Our mission at Southside Church is to lead people into a growing relationship with Jesus Christ. This first section about your spiritual history helps us to learn about your growing relationship with Jesus. Please be as complete and concise as possible.
